Obtain login credentials from us here at Jarvis Vision Center following your appointment
Go to https://revolutionphr.com/portal/welcome.htm
Enter those credentials in the space provided in the upper right corner
Click “Login”

This should take you to your online personal health record. If there is any errant or missing information, please contact us to let us know.
